Authors' conclusion

A bleeding risk score can be used to stratify patients with POAF after CABG into groups with different post-discharge bleeding risk. Further studies are necessary to identify the optimal risk score and its role in oral anticoagulation decision pathway to improve clinical outcomes.

Study context

Population
Patients with new-onset post-operative atrial fibrillation (POAF) after coronary artery bypass grafting (CABG) in 2009–2020 without oral anticoagulationpatients with POAF after CABG without oral anticoagulation
Participants
4,436 patients
Exposure
Associations between bleeding risk and major bleeding events during the first post-operative year

Main finding

As reported in the abstract

Major bleeding · first year

Major bleeding, all patients

2.1%

Observational data: associations, not effects.

Further findings

Outcomehazard ratio (95% CI)
Major bleeding by bleeding risk category · first post-operative year4.81 (2.59–8.96)

High bleeding risk classification

Patients classified as high bleeding risk
36.0%

Discrimination

Area under the receiver operating characteristic curve
0.68 (95% CI 0.63-0.73)

Calibration

Good agreement between expected and observed bleeding events in patients with an annual bleeding risk up to 7%
