Authors' conclusion

Reduction in RVEDV with increase in LVEDV following TTVR is associated with reduction in LV eccentricity with an increase in forward SV. CT measures of RV function (effective RVEF) and RV coupling (SV/RVESV) both improve following TTVR.

Study context

Population
Patients undergoing transcatheter tricuspid valve replacement (TTVR) for tricuspid regurgitation50 with paired computed tomography (CT)
Participants
80 patients
Outcome
Biventricular structural and functional changes after TTVR (echocardiography and CT)

Main finding

As reported in the abstract

CT median 40 days post-TTVR
Increase in effective RVEF
65.3%
Increase in right ventricular (RV) coupling
20.3%
Effective RVEF and RV coupling increase, P
P < 0.001
RVEDV decreased, mL/m² (pre to post)
138.2 to 59.5
RVEDV decrease, P
P < 0.001
LVEDV increased, mL/m² (pre to post)
49.6 to 57.9
LVEDV increase, P
P = 0.001
Septal curvature increased and left ventricle (LV) aEI decreased
P < 0.001

Observational data: associations, not effects.

Further findings

Tricuspid regurgitation

Baseline massive/torrential TR
87.5%
≤mild TR post-implantation
95.9%

Echocardiographic RV function, SV and cardiac index

All echocardiographic measures of RV function reduced
P < 0.001
Increase in SV and cardiac index
P < 0.001

Baseline aEI ≥1.25 vs low aEI (subgroup)

Greater reverse remodeling, lower follow-up NT-proBNP and greater symptom improvement with baseline aEI ≥1.25
