Differences in HDL-related mortality risk between individuals with and without hypertension: a prospective cohort study in UK Biobank

Eur J Prev CardiolPublished

  • Prospective cohort study

Authors' conclusion

The increased risk of mortality associated with very high HDL-C existed only in individuals with hypertension, but not in those without hypertension. Moreover, the increased risk at high HDL-C levels in hypertension was likely driven by larger HDL-P.

Study context

Population
UK Biobank participants with and without hypertension244,866 with hypertension
Participants
429,792 participants
Exposure
Very high vs normal high-density lipoprotein cholesterol (HDL-C), stratified by hypertension
Outcome
All-cause mortality
Follow-up
Median 12.7 years

Main finding

As reported in the abstract

All-cause mortality, by hypertension (subgroup analysis)
Very high HDL-C vs Normal HDL-CHazard ratio (95% CI)
with hypertension
1.47(1.35–1.61)
without hypertension
1.05(0.91–1.22)

Observational data: associations, not effects.

Further findings

Deaths, by hypertension · median follow-up 12.7 years

HDL particle size and all-cause mortality (hypertension)

Larger HDL particle (HDL-P) positively, smaller HDL-P negatively associated with all-cause mortality
After adjustment for larger HDL-P, U-shaped HDL-C association became L-shaped
