Diuretic Resistance and the Efficacy of Hydrochlorothiazide in Acute Decompensated Heart Failure: A Post-Hoc Analysis of the CLOROTIC Trial

Eur J Heart FailPublished

  • Post-hoc analysis of the CLOROTIC randomised trial

Authors' conclusion

The BAN-ADHF score demonstrated good performance in identifying DR, and high DR risk was associated with worse outcomes. HCTZ treatment response on weight loss was similar across DR risk, but improvement in dyspnoea with HCTZ was attenuated in high DR risk.

Study context

Population

Participants
220 patients
Comparison
Hydrochlorothiazide (HCTZ) vs placebo across diuretic resistance risk

Primary result

As reported in the abstract

HCTZ vs placebo treatment effect by DR risk

HCTZ effect on weight change similar across diuretic resistance (DR) risk, 72 h

pint = 0.75

HCTZ effect on weight change similar across DR risk, 96 h

pint = 0.50

HCTZ improvement in dyspnoea visual analogue scale (VAS) AUC, high vs low DR risk, 72 h

−236 vs. 476 mm; pint = 0.03

HCTZ improvement in dyspnoea VAS AUC, high vs low DR risk, 96 h

−400 vs. 777 mm; pint = 0.001
