Diurnal variation in left atrial pressure in ambulatory patients with chronic heart failure
- Prospective observational cohort study
- implantable intracardiac left atrial pressure (LAP) sensor
- longitudinal follow-up
Authors' conclusion
Using direct ambulatory intracardiac LAP monitoring, we observed consistent diurnal variation with higher evening pressures, largely unchanged by diuretic dose or dosing schedule, atrial fibrillation, or association with ADHF events.
Study context
- Population
- Ambulatory patients with chronic HF with an implantable intracardiac LAP sensor>15,500 measurement days
- Participants
- 73 patients
- Exposure
- Evening vs morning LAP
- Outcome
- Diurnal variation in LAP (morning vs evening)
- Follow-up
- Average 25.5±18.6 months

|---|---|---|---|---|
| Higher LAP and risk of ADHF events · independently associated | 1.40 (1.10–1.78) | P < 0.01 | ||

| Outcome | Values | Estimate | P value |
|---|---|---|---|
| LAP in patients with vs without ADHF events | ADHF events median 14.3 (IQR 9.5–19.9) mmHg · No ADHF events median 10.4 (IQR 5.4–17.3) mmHg | P < 0.01 |
Diurnal variation by diuretic dose, twice-daily dosing or AF
- Neither higher diuretic dose, twice-daily dosing, nor AF modified the diurnal variation
- P = 0.98, p=0.50
Diurnal variation by acute decompensated heart failure (ADHF) status
- Diurnal variation present irrespective of ADHF status
- P = 0.79
