Effect of respiratory syncytial virus (RSV) vaccine in older adults with atrial fibrillation: a prespecified analysis of the DAN-RSV trial
- Secondary analysis of the DAN-RSV randomised trial
- prespecified analysis
- pragmatic
- open-label
- individually randomized
- registry-based data
Authors' conclusion
In older adults, RSVpreF vaccine was associated with a lower incidence of RSV-related and all-cause respiratory tract disease hospitalizations, as well as RSV-related and all-cause cardio-respiratory disease hospitalizations, with no evidence of effect modification by AF status.
Study context
- Population
- Adults ≥60 years in Denmark, 2024/2025 winter season, with and without atrial fibrillation (AF)10,126 (7.7%) with AF
- Participants
- 131,276 adults
- Comparison
- RSVpreF vaccine vs no vaccine, by AF status
- Primary endpoint
- Hospitalization for RSV-related respiratory tract disease
- Follow-up
- 14 days after study visit until May 31, 2025
Primary result
As reported in the abstract
RSV-related respiratory tract disease hospitalization, by AF status (subgroup)
Vaccine effectiveness (VE)
- AF
- 80.6% (95% CI −84.7 to 99.6)
- no AF
- 84.6% (95% CI 32.3–98.3)
Further findings
| Outcome | VE (95% CI) |
|---|---|
| All-cause respiratory tract disease hospitalization, by AF status — RSVpreF vaccine vs No vaccine · AF | 25.7% (−16.9 to 46.1) |
| All-cause respiratory tract disease hospitalization, by AF status — RSVpreF vaccine vs No vaccine · no AF | 13.0% (−3.3 to 27.8) |
RSV-related and overall cardio-respiratory disease hospitalization
- Lower incidence with vaccine, no effect modification by AF status
- interaction p≥0.45 for all
AF or stroke hospitalization
- Not significantly associated with vaccine regardless of AF status
- interaction p≥0.34 for all
