Frailty, Pitavastatin, and Major Adverse Cardiovascular Events Among People With HIV
Authors' conclusion
Frailty was associated with markedly higher hazard of MACE. Though frailty did not appear to modify the protective effects of pitavastatin seen in the primary trial, the efficacy in frail PWH remains uncertain due to the limited number of frail individuals.
Study context
- Population
- People with HIV (PWH) without ASCVD, aged 40 to 75 yearshad sufficient data to calculate frailty index, of 7769 REPRIEVE participants
- Participants
- 7,740 participants
- Comparison
- Major adverse cardiovascular events (MACE) hazard by frailty status (prefrail and frail compared to nonfrail); pitavastatin 4 mg vs placebo effect modification by frailty status
Primary result
As reported in the abstract
MACE · median follow-up was 5.6 years
Each group vs nonfrailHazard ratio (95% CI)
Prefrail
1.76(1.35–2.30)
Frail
2.14(1.33–3.45)
- MACE hazard increased with frailty
- P < 0.0001

Baseline characteristics
- Median age
- 50 years (Q1, Q3: 45, 55)
- Nonfrail
- 67%
- Prefrail
- 29%
- Frail

Pitavastatin effect modification by frailty status
- No evidence that pitavastatin effect differed by frailty status
- P = 0.44
