Health Status Across Major Subgroups of Patients with Heart Failure and Preserved Ejection Fraction
- Secondary analysis of the EMPEROR-Preserved randomised trial
- subgroup analysis
Authors' conclusion
Health status in patients with HFpEF is impaired, especially in elderly, women, and those with obesity and comorbidities. Empagliflozin improved health status among all key subgroups studied with a greater effect in obese patients.
Study context
- Population
- Patients with heart failure and preserved ejection fraction (HFpEF)
- Comparison
- Empagliflozin vs placebo
- Placebon = 2,979reference
- Primary endpoint
- Kansas City Cardiomyopathy Questionnaire (KCCQ) scores (baseline, change over time, empagliflozin effect) across subgroups
- Follow-up
- Baseline, 12, 32 and 52 weeks
Primary result
As reported in the abstract
KCCQ-CSS at baseline (placebo) · baseline
Placebo
70.7
mean · SD 20.8
- KCCQ-CSS improved during follow-up; less improvement with AF/flutter at enrollment
- P for trend = 0.014
- Less KCCQ-CSS improvement with chronic kidney disease (CKD)
- P for trend < 0.001
- Empagliflozin improved KCCQ-CSS, -TSS and -OSS similarly across subgroups except greater improvement with highest body mass index (BMI)
- P for trend = 0.153, 0.08 and 0.078, respectively
Further findings
5-point increase in KCCQ-CSS at week 12 and subsequent outcomes
- Lower risk of CV death or HF hospitalization
- 5%
- Lower risk of CV death
- 8%
- Lower risk of first HF hospitalization
- 4%
- similar trend with KCCQ-TSS and overall summary score (OSS)
