Increased Platelet FcɣRIIa Identifies Patients at Greater Risk of Multiple Cardiovascular Events After Myocardial Infarction

JACC AdvPublished

  • prospective noninterventional trial

Authors' conclusion

High pFCG is associated with a greater risk of both first and additional ischemic endpoints. The strong association of pFCG with a greater risk of recurrent ischemic events may be useful to inform clinical decision-making.

Study context

Population
Patients with type 1 myocardial infarction (MI) with at least 2 of: age ≥65 years, multivessel coronary artery disease, prior MI, chronic kidney disease, and diabetespatients with type 1 MI enrolled · no events 562 · bleeding events alone 45 · single ischemic event 101 · bleeding + ischemic events 29 · multiple ischemic events 23 · fatal MI 5
Participants
765 patients
Exposure
pFCG in patients with and without subsequent ischemic and bleeding events
  • no eventsn = 562
  • bleeding events alonen = 45
  • single ischemic eventn = 101
  • bleeding + ischemic eventsn = 29
  • multiple ischemic eventsn = 23
  • fatal MIn = 5

Main finding

As reported in the abstract

Average pFCG (molecules/platelet) by patient group

Observational data: associations, not effects.

Further findings

OutcomeValues
Average pFCG expression in patients with bleeding (without ischemia)bleeding events alone average 1,491 ± 517 molecules/platelet

Ischemic events associated with high pFCG (accentuated by additional ischemic endpoints) · between 6 months and 1 year

  • Early ischemic events associated with high pFCG were driven by first ischemic endpoints

Average pFCG expression in patients with bleeding (without ischemia)

similar to that in patients without ischemia or bleeding
