Influence of Chronic Kidney Disease on Outcomes After Paclitaxel-Coated Balloon Angioplasty for Coronary In-Stent Restenosis

Circ Cardiovasc IntervPublished

  • post hoc analysis of the AGENT IDE trial
  • time-to-first-event and recurrent event methods

Authors' conclusion

Among patients undergoing intervention for in-stent restenosis, the benefit of DCB versus balloon angioplasty was consistent irrespective of baseline CKD status. Baseline renal function also did not influence outcomes in the recurrent event analysis.

Study context

Population
Patients undergoing intervention for coronary in-stent restenosis, with chronic kidney disease (CKD) (estimated glomerular filtration rate <60 mL/min per 1.73 m 2 ) and without renal impairmentpatients randomized to drug-coated balloons (DCB), 149 (25%) had CKD · CKD 149
Participants
406 patients
Comparison
DCB vs balloon angioplasty
Primary endpoint
Target lesion failure:
  • Ischemia-driven target lesion revascularization (TLR)
  • Target vessel myocardial infarction
  • Cardiac death
Follow-up
2 years

Primary result

As reported in the abstract

DCB vs balloon angioplastyHazard ratio (95% CI)
patients with CKD: 23.6% versus 30.0%
0.70(0.35–1.39)
patients without CKD: 28.1% versus 34.9%
0.74(0.52–1.05)

Further findings

Baseline characteristics of CKD patients

mean estimated glomerular filtration rate (CKD)
47.3±9.2
Older, greater multivessel disease and congestive heart failure than those without CKD
No differences in clinical presentation, Mehran in-stent restenosis patterns or technical success

Recurrent events analysis

Effect of DCB not influenced by baseline renal function
