Intensive early and sustained lowering of non–high-density lipoprotein cholesterol after myocardial infarction and prognosis: the SWEDEHEART registry

Eur Heart JPublished

  • Registry-based cohort study

Authors' conclusion

The lowest achieved levels both at 2 months and at 1 year of non-HDL-C were associated with better outcome. The lowest risk was observed when target was achieved within 2 months of MI and sustained thereafter.

Study context

Population
Patients with myocardial infarction (MI) in the SWEDEHEART registry
Participants
56,262 patients
Exposure
Lowest vs highest quartile of achieved non-HDL-C after MI
Outcome
MACE:
  • Death
  • MI
  • Ischaemic stroke
Follow-up
Median 5.4 years

Main finding

MACE by achieved non-HDL-C at 1 year

Hazard ratio

0.76

95% CI 0.71–0.81

Lowest quartile vs Highest quartile
HR 0.76 (95% CI 0.71–0.81)

As reported in the abstract

MACE events
9549

Observational data: associations, not effects.

Further findings

MACE by non-HDL-C level at 2 months, incl. early events up to 1 year

Short-term results consistent with the 1-year analysis
HR 0.80, 95% CI 0.68-0.92

Outcomes by timing of non-HDL-C target achievement (<2.2 mmol/L)

Early and sustained target achievement (lowest risk)
HR 0.80, 95% CI 0.74-0.86
Target achieved early or late
HR 0.86, 95% CI 0.79-0.93

Death and non-fatal MI

Similar results observed for all outcomes
