Authors' conclusion

Defect-free care adherent to the performance measures was associated with improved long-term clinical outcomes, including lower mortality, among patients with STEMI undergoing percutaneous coronary intervention, whereas no association was observed for repeat coronary revascularization.

Study context

Population
Patients with STEMI who underwent percutaneous coronary intervention within 24 hours after symptom onset and survived to discharge (CREDO-Kyoto AMI Registry Wave-2)patients with STEMI who underwent PCI within 24 hours and survived to discharge · PMs=9 551 · PMs=8 985 · PMs=7 794 · PMs≤6 562
Participants
2,892 patients
Exposure
Adherence to 9 guideline-based performance measures for STEMI care, by number of fulfilled performance measures, relative to PMs=9
  • PMs=9n = 551reference
  • PMs=8n = 985
  • PMs=7n = 794
  • PMs≤6n = 562

Repeat coronary revascularization — PMs=8 vs PMs=91.00 (0.82–1.21)
Repeat coronary revascularization — PMs=7 vs PMs=90.89 (0.72–1.09)
Repeat coronary revascularization — PMs≤6 vs PMs=90.91 (0.72–1.15)

Most clinical outcomes (secondary)

Inverse associations between adherence to performance measures and most clinical outcomes except repeat coronary revascularization
