Mitral annular disjunction and mitral valve prolapse: long-term risk of ventricular arrhythmias after surgery

Eur Heart JPublished

  • Retrospective cohort study
  • single-centre

Authors' conclusion

Mitral annular disjunction in patients with MVP and MR was associated with a three-fold increased long-term risk of VA post-mitral valve surgery, despite anatomical correction of MAD.

Study context

Population
Patients with mitral valve prolapse (MVP) and moderate or severe degenerative mitral regurgitation (MR) undergoing mitral valve surgery (repair or replacement), Karolinska University Hospital, 2010–22
Participants
599 patients
Exposure
Pre-operative mitral annular disjunction (MAD) vs no MAD
  • MADn = 96
Outcome
Ventricular arrhythmias (VA) (hospitalizations, outpatient visits or ablation for confirmed sustained or non-sustained VT, or high burden of premature ventricular complexes)
Follow-up
Median 5.4 (IQR 2.8-7.5) years

Main finding

VA

Hazard ratio

3.33

95% CI 1.37–8.08

MAD vs No MAD
HR 3.33 (95% CI 1.37–8.08)

As reported in the abstract

Observational data: associations, not effects.
