Coronary disease & ACSCohort study
National Trends and Outcomes of Acute Myocardial Infarction After Transcatheter Aortic Valve Replacement
- Retrospective cohort study
- propensity-score matched
- national database
Authors' conclusion
There were no differences in the use of coronary revascularization for STEMI or NSTEMI in TAVR patients compared with the non-TAVR population. In-hospital mortality for STEMI is higher in TAVR patients compared with those without prior TAVR.
Study context
- Population
- Patients with STEMI or NSTEMI with or without prior transcatheter aortic valve replacement (TAVR), US Vizient Clinical Data Base, 2016–2022
- Participants
- 206,229 TAVR patients
- Exposure
- Prior TAVR vs no TAVR in patients with STEMI or NSTEMI
- Outcome
- Use of coronary angiography, revascularization and in-hospital outcomes
Main finding
As reported in the abstract
Coronary revascularization, by AMI type
- Use of coronary revascularization similar in prior TAVR and no TAVR cohorts
- Prior TAVR vs No TAVR: P = 0.81
- Prior TAVR vs No TAVR: P = 0.88
- after propensity matching
Observational data: associations, not effects.
Further findings
In-hospital mortality, STEMI (subgroup) · propensity matched
P = 0.03
In-hospital mortality, NSTEMI (subgroup) · propensity matched
P = 0.02
Incidence of acute myocardial infarction (AMI) after TAVR
- STEMI incidence
- 25 events per 100,000 person-years
- NSTEMI incidence
- 229 events per 100,000 person-years
- n = 206,229 TAVR patients
