Heart failure & cardiomyopathiesCohort study

Physical Activity and Cardiovascular Outcomes in Phenotype-Negative Cardiomyopathy Variant Carriers

JAMA CardiolPublished

  • Prospective cohort study
  • 22 assessment centres, UK Biobank

Conducted February 2013 to December 2015; analysed March 2024 to June 2025

Authors' conclusion

In this cohort study, MVPA within the general range of guideline-based recommendations was associated with lower risk of adverse CV outcomes and similar degrees of cardiac remodeling for G+P- carriers compared to noncarriers.

Study context

Population
UK Biobank participants with whole-genome sequencing and 1 week of accelerometer data, without prevalent heart failure (HF), atrial fibrillation (AF), cardiomyopathy, ventricular arrhythmias (VAs) or ICDs
Participants
84,699 participants
Exposure
Higher vs zero accelerometer-measured moderate to vigorous physical activity (MVPA), G+P- carriers vs noncarriers
  • G+P- carriersn = 3,979
Outcome
Incident adverse CV outcomes (AF, HF, MI, stroke), CMR measures of cardiac remodeling, cardiomyopathy and VA, by MVPA volume
Follow-up
Median 8.0 years

Main finding

As reported in the abstract

Incident CV disease in G+P- carriers (optimal MVPA vs zero) · median (IQR) 8.0 (7.5-8.5) years
Optimal MVPA vs Zero MVPAHazard ratio (95% CI)
AF, G+P- carriers
0.68(0.58–0.79)
HF, G+P- carriers
0.58(0.47–0.71)
MI, G+P- carriers
0.49(0.24–1.00)
stroke, G+P- carriers
0.35(0.12–0.99)

Further findings

OutcomeHR (95% CI)
Incident cardiomyopathy in G+P- carriers0.03 (0.00–0.98)

Cardiac magnetic resonance (CMR) cardiac remodeling (eg, LV dilation, LV hypertrophy)

Similar pattern and extent of remodeling with MVPA in G+P- carriers vs noncarriers

Variant subtypes (DCM, HCM, ARVC)

Findings generally consistent across variants; limited precision for ARVC

Safety

OutcomeHR (95% CI)
VA among G+P- carriers · no increase in risk of VA0.98 (0.83–1.14)
