Predictive value of ambulatory ECG monitoring for malignant arrhythmic events in genetic dilated cardiomyopathy
- Cohort study
Authors' conclusion
Ambulatory ECG monitoring abnormalities are prevalent in genetic DCM patients. In contrast to non-genetic DCM patients, ambulatory ECG parameters have an important predictive value to determine the risk of MVAE in genetic DCM patients.
Study context
- Population
- Recently diagnosed, genotyped dilated cardiomyopathy (DCM) probands
- Participants
- 354 genotyped DCM probands
- Exposure
- Genetic vs non-genetic DCM
- Genetic DCMn = 123
- Outcome
- MVAE (VF, sustained VT, anti-tachy pacing, appropriate device therapy or SCD)
- Follow-up
- Median 8 years (IQR: 5-9 years)
Main finding
As reported in the abstract
Prediction of MVAE by ambulatory ECG variables (perAF, AV-block, NSVT, >500 premature ventricular complexes (PVCs)/24 h)
| Group | AUC |
|---|---|
| Genetic DCM | 0.768 |
| Non-genetic DCM | 0.628 |
- Genetic DCM vs Non-genetic DCM: P = 0.044
Observational data: associations, not effects.
Further findings
Abnormalities on ambulatory ECG
P = 0.013
| Outcome | Genetic DCM | Non-genetic DCM | P value |
|---|---|---|---|
| Incremental predictive value of clinical variables | |||
| AUC Δ | +0.004 | +0.150 |
Permanent AF, paroxysmal SVT, non-sustained ventricular tachycardia (NSVT)
- More frequent in genetic DCM (permanent atrial fibrillation (perAF), paroxysmal supraventricular tachycardia (parox-SVT), NSVT)
- P = 0.041, <0.001, and <0.001
Structural cardiac parameters
- Minimal group differences
