Prognostic Utility of the HFpEF-ABA Score for Heart Failure with Preserved Ejection Fraction (HFpEF) Patients
- Cox proportional hazards and competing risk regression analyses
Authors' conclusion
Our study demonstrates substantial regional heterogeneity in the prognostic utility of the HFpEF-ABA score among patients with HFpEF enrolled in the TOPCAT trial. The score exhibited strong associations with adverse outcomes in the Russia/Georgia cohort, whereas its prognostic discrimination was attenuated in the Americas cohort. (Aldosterone Antagonist Therapy for Adults With Heart Failure and Preserved Systolic Function [TOPCAT]; NCT00094302).

Further findings
Clinical outcomes, Americas cohort
| Comparison | HR per 10% increase (95% CI) | |
|---|---|---|
| HFpEF-ABA probability · HF hospitalization | 1.07 (1.00–1.14) | |
| HFpEF-ABA probability · any hospitalization | 1.04 (1.00–1.08) | |
| 0.512Hr per 10% increase (log scale) |
Clinical outcomes, Russia/Georgia cohort
| Comparison | HR (95% CI) | |
|---|---|---|
| Higher HFpEF-ABA probabilities · all-cause mortality | 1.67 (1.16–2.41) | |
| Higher HFpEF-ABA probabilities · heart failure hospitalization | 1.98 (1.04–3.78) | |
| Higher HFpEF-ABA probabilities · any hospitalization | 1.75 (1.43–2.14) | |
| Higher HFpEF-ABA probabilities · stroke | 2.81 (1.44–5.5) | |
| 0.20.5125Hazard ratio (log scale) |
Clinical outcomes, Americas cohort
- without significant associations with mortality or major cardiovascular events
Clinical outcomes, Russia/Georgia cohort
- A linear trend of escalating adverse outcome risks corresponded with increasing HFpEF-ABA probabilities in the Russia/Georgia cohort, excluding stroke
