Proteomic Profiles of Left Atrial Volume and Its Influence on Response to Spironolactone: Findings from the HOMAGE Trial and STANISLAS Cohort

Eur J Heart FailPublished

  • Secondary analysis of the HOMAGE randomised trial with external replication in the STANISLAS population-based cohort

Authors' conclusion

In individuals without heart failure, LAVi was associated with MMP-2, IGFBP-2 and NT-proBNP. Spironolactone reduced these biomarker concentrations as well as LAVi and PICP, irrespective of left atrial size.

Study context

Population
People at risk of HF (HOMAGE); asymptomatic individuals from a population-based cohort (STANISLAS)HOMAGE analysed
Participants
421 people
Comparison
Spironolactone vs control; association of LAVi with circulating proteins
Primary endpoint
Clinical and proteomic characteristics associated with LAVi, and whether LAVi influences response to spironolactone

Primary result

As reported in the abstract

Proteins associated with greater LAVi (HOMAGE)

Greater LAVi associated with higher MMP-2, IGFBP-2 and NT-proBNP after adjustment and multiple-testing correction

FDR <0.05

Associations externally replicated in STANISLAS

all FDR <0.05

Further findings

Effect of spironolactone on MMP-2 and NT-proBNP, by baseline LAVi

Spironolactone decreased MMP-2 and NT-proBNP regardless of baseline LAVi
P for interaction > 0.10

Effect of spironolactone on LAVi, LVEF, E/e', BP and PICP

Spironolactone significantly reduced LAVi, improved LVEF, lowered E/e', BP and PICP regardless of baseline LAVi
P for interaction > 0.10
