Acute cardiac care & resuscitation

A randomized trial of expedited intra-arrest transfer versus more extended on-scene resuscitation for refractory out of hospital cardiac arrest: Rationale and design of the EVIDENCE trial

Am Heart JPublished Randomised controlled trial

Study context

Population
Adults aged 18 to 70 with witnessed refractory OHCA, assumed medical cause, immediate bystander cardiopulmonary resuscitation (CPR), initial ventricular fibrillation (VF)/pulseless ventricular tachycardia (VT) or Pulseless Electrical Activity (PEA), no ROSC after 3 shocks and/or 15 minutes of professional on-scene resuscitation, mechanical CPR available; 2 urban regions in NSW Australiaplanned, 1:1
Participants
200 adults
Planned comparison
Expedited transport from scene vs ongoing advanced life support (ALS) on-scene
Primary endpoint
Survival to hospital discharge with cerebral performance category (CPC) 1 or 2
Follow-up
Hospital discharge
Methodological detailsrationale and design, phase III, multicentre, partially blinded, prospective, intention-to-treat, contemporaneous registry of ineligible patients, Secondary outcomes: safety, survival, prognostic factors, ECMO-supported CPR, functional assessment at discharge, 4 weeks and 6 months, quality of life, healthcare use, cost-effectiveness
