Rationale and design of the direct oral anticoagulants for prevention of left ventricular thrombus after anterior acute myocardial infarction (APERITIF) trial

Am Heart JPublished Design paper of a randomised controlled trial

Study context

Population
Patients with anterior ST-elevation-myocardial infarction (STEMI) or very high-risk non- ST-elevation-myocardial infarction (NSTEMI) with involvement of the left anterior descending artery, admitted to cardiac Intensive Care Unit (ICU) within 48 hours of symptom onsetplanned sample size 560
Planned comparison
Dual Antiplatelet Therapy (DAPT) plus rivaroxaban 2.5mg twice daily vs DAPT alone
Primary endpoint
Presence of LV thrombus detected by contrast enhanced CMR
Follow-up
1 month
Methodological detailsAPERITIF, investigator-initiated, multicentre, open-label, blinded end-point (PROBE), nested in the FRENCHIE registry, Secondary endpoints: Left ventricular (LV) thrombus dimension (greatest diameter), major bleedings and major CV events at 1 month, Sample size of 560 patients for 80% power to show superiority
