Renal replacement therapy in type 2 severe cardiorenal syndrome: a randomized controlled trial
- multicentre, randomized, controlled, open-label trial
- 15 French centres
Authors' conclusion
In patients with type 2 CRS and refractory congestive HF, adding ultrafiltration through PD, HD, or IUF did not significantly reduce mortality or HF-related hospitalizations at 12 months compared with optimized medical therapy and close multidisciplinary follow-up, although the trial was underpowered.
Study context
- Population
- Adults with severe heart failure (HF), persistent or recurrent congestion despite high-dose diuretics and guideline-directed medical therapy, and mild to severe chronic kidney disease (CKD) (type 2 cardiorenal syndrome)randomized, 108 screened · Control group 24 · Ultrafiltration group 22
- Participants
- 46 adults
- Comparison
- Ultrafiltration group vs Control group
- Ultrafiltration groupn = 22
- Control groupn = 24
- Primary endpoint
- Composite of:
- All-cause mortality
- Unplanned hospitalization for acute HF
- Follow-up
- Within 12 months (median 262 days)
Primary result
As reported in the abstract
P = 0.144
Further findings
Quality-of-life scores
- Seemed to improve over time in both groups, with a slightly more sustained improvement in the ultrafiltration group
Safety
