Sacubitril/valsartan and cardiac injury/stress and systemic inflammatory biomarkers in acute heart failure: a pre-specified secondary analysis of the PREMIER study
- multicentre, prospective, randomized, open-label trial with blinded-endpoint assessment
- pre-specified secondary analysis
Authors' conclusion
In haemodynamically stabilized patients hospitalized for AHF, no differential effects of Sac/Val vs. continued ACEI/ARB therapy on cardiac injury/stress and systemic inflammatory biomarkers were demonstrated over 8 weeks. These findings do not support a broad biomarker-defined anti-inflammatory or cardioprotective effect of Sac/Val in this setting.
Study context
- Population
- Inpatients hospitalized for acute heart failure, previously treated with ACEI/ARB, randomized within 7 days of hospitalization after haemodynamic stabilizationpatients with complete cardiac injury/stress and systemic inflammatory biomarker data
- Participants
- 365 inpatients
- Comparison
- Sac/Val vs. continued ACEI/ARB therapy
Primary result
As reported in the abstract
Change in high-sensitivity cardiac troponin T (hs-cTnT), soluble ST2 (sST2), CRP and GDF-15 · 8 weeks
- all biomarkers generally decreased over 8 weeks, but between-group differences in biomarker changes were not significant
Further findings
NT-proBNP by baseline C-reactive protein (CRP) (exploratory biomarker-stratified analysis)
- only baseline CRP showed a nominal treatment interaction for NT-proBNP
- P for interaction = 0.012
- exploratory
