SCORE2-OP-guided statin initiation for primary prevention in adults aged 70–89 years without diabetes: evidence for age-specific risk thresholds
- Retrospective cohort study
- observational
- intention-to-treat
- propensity-adjusted
Authors' conclusion
Age-specific risk thresholds of the SCORE2-OP estimated risk may be further developed and considered when weighing the benefits of initiating primary prevention of incident CVD with statins in non-diabetic adults aged 70+. Our results do not support treatment initiation of primary prevention to reduce all-cause mortality at any level of estimated CVD risk.
Study context
- Population
- Non-diabetic adults aged 70–89, Catalan primary care database (SIDIAP)36.9% men
- Participants
- 167,360 adults
- Exposure
- Statin initiation vs no statin initiation for primary prevention
- Outcome
- Incident CVD (MI and stroke), by SCORE2-OP risk
Main finding
As reported in the abstract
Overall: statin treatment not associated with cardiovascular disease (CVD) reduction at lower risks; protective association stable around SCORE2-OP of 16%
HR=0.90 [0.84-0.96]
Age groups: similar protective associations, but at different estimated risks
HR ≈ 0.85
Observational data: associations, not effects.
Further findings
All-cause mortality
- No protective association across the whole estimated risk spectrum
Safety
| Outcome | Estimate |
