Valvular & structural heart diseaseCohort study
Severe tricuspid regurgitation: aetiology, patient characteristics, and treatment: a multicentre, prospective registry
- prospective, observational study
- multicentre
- 18 cardiology centres
- consecutive patients
Authors' conclusion
In real life, unequivocal identification of TR aetiology remains challenging. Secondary atrial TR is the most common aetiology. There are significant differences in characteristics and outcomes depending on TR aetiology. Too few patients with severe TR undergo evaluation by the Heart Team.
Study context
- Population
- Consecutive adult patients with severe tricuspid regurgitation, regardless of tricuspid regurgitation (TR) symptoms and cause of hospital admissionpatients with severe TR enrolled (median age 76 years, 53% women)
- Participants
- 1,295 patients
- Exposure
- Patient characteristics and treatment by TR aetiology
Main finding
As reported in the abstract
Reason for admission
Heart failure (HF) decompensation (most common reason for admission)
40%
Observational data: associations, not effects.
Further findings
TR aetiology
years
| Outcome | secondary atrial | secondary ventricular | Primary | CIED-related |
|---|---|---|---|---|
| Age by aetiology | 79 | Not reported in the abstract | 68 | 78 |
- TR aetiology — Single TR aetiology identified
- 79%
- TR aetiology — Overlap of secondary ventricular and atrial TR
- 11%
- TR aetiology — TR aetiology not determined
- 2.4%
Clinical characteristics and in-hospital mortality by aetiology
- Patients with cardiac implantable electronic device (CIED)-related and secondary ventricular TR were more often hospitalized for HF decompensation, had more advanced HF symptoms, worse left- and right-ventricular function, worse kidney and liver function, and higher in-hospital mortality
Heart Team evaluation and qualification for TR interventions
- Evaluated by the Heart Team
- 40%
- Qualified for TR interventions
- 21%
