Sleep-Related Hypoxia, Right Ventricular Dysfunction, and Survival in Patients With Group 1 Pulmonary Arterial Hypertension

J Am Coll CardiolPublished

  • Prospective cohort study
  • multicentre cohort (PVDOMICS)

Authors' conclusion

Sleep-related hypoxia was more strongly associated than AHI with measures of RV dysfunction, death, or transplantation overall and in group 1 non-CTD-associated PAH but only with RV dysfunction in CTD-associated PAH.

Study context

Population
Patients with group 1 pulmonary arterial hypertension (PAH) with sleep studies, PVDOMICS cohort43 events
Participants
186 patients
Exposure
Sleep-related hypoxia and apnea-hypopnea index (AHI), per 10-unit increment
Outcome
RV function measures and transplantation-free survival in association with AHI and T90 (per 10-unit increment)

Main finding

As reported in the abstract

Association with RV function and transplantation-free survival, per 10-unit increment
Change in RV ejection fraction (MRI), % (95% CI)P value
AHI
−2.18(−4.00 to −0.36)
P = 0.019
T90
−0.93(−1.47 to −0.40)
P < 0.001

Per 10-unit increment, as printed

Estimate (95% CI)P value
T90
change in RV systolic pressure(echocardiography) 2.52 mm Hg (1.61–3.43)
P < 0.001
T90
change in mean pulmonary artery pressure(right heart catheterization) 0.27 mm Hg (0.05–0.49)
P = 0.019

Per 10-unit increment, as printed

Hazard ratio (95% CI)
T905-year transplantation or death
1.17(1.07–1.28)

Per 10-unit increment, as printed

T90
Right ventricular (RV) hypertrophy (ECG) 1.24 mm (95% CI 1.10–1.40); P < 0.001

Observational data: associations, not effects.

Further findings

Transplantation or death, non-CTD-associated PAH (subgroup)

ComparisonHR (95% CI)
T901.21 (1.08–1.34)

Connective tissue disease (CTD)-associated PAH (subgroup)

T90 associated with RV dysfunction, but not death or transplantation
