Cohort study
Thrombus Burden and Outcomes in Patients With COVID-19 Presenting With STEMI Across the Pandemic
- Observational cohort study
- single-centre
- consecutive patients
Authors' conclusion
Significant changes have occurred in the clinical characteristics, angiographic features, and outcomes of STEMI patients with COVID-19 infection treated by primary percutaneous coronary intervention during the course of the pandemic. Importantly, outcomes of recent waves and in vaccinated individuals are no different to a non-COVID-19 population.
Study context
- Population
- Consecutive patients admitted with confirmed STEMI treated with PCI, UK, January 3, 2020 to October 3, 2022
- Participants
- 1,269 patients
- Exposure
- COVID-19-positive vs COVID-19-negative
- COVID-19-positiven = 154
- COVID-19-negativen = 1,115reference
Main finding
As reported in the abstract
Clinical characteristics and outcomes across waves
- Wave 1: COVID-19-positive STEMI patients had high rates of cardiac arrest, increased thrombus burden, bigger infarcts and worse outcomes
- Wave 3: no differences in outcomes between COVID-19-positive and -negative patients, with significant differences vs earlier COVID-19-positive patients
- Poor outcomes later in the study period predominantly in unvaccinated individuals
Observational data: associations, not effects.
