Transcatheter Tricuspid Valve Replacement in Patients With Cardiac Implantable Electronic Device Leads
- multicenter TRIPLACE (Global Multicenter Registry on Transcatheter Tricuspid Valve Replacement) registry
- All consecutive patients
Authors' conclusion
Orthotopic TTVR in patients with transvalvular CIED leads can be safely and effectively performed with low rates of lead failure. Significant paravalvular leak and residual tricuspid regurgitation is more common with a jailed lead. These patients require close CIED follow-up with alternative pacing strategies in place, particularly when pacing dependent. (Global Multicenter Registry on Transcatheter TRIcuspid Valve RePLACEment [TRIPLACE]; NCT06033274).
Study context
- Population
- Patients undergoing orthotopic transcatheter tricuspid valve replacement (TTVR), stratified by presence of a cardiac implantable electronic device (CIED) lead traversing the tricuspid valve104 (26.3%) had transvalvular CIED leads
- Participants
- 395 patients
- Exposure
- Patients with vs without a CIED lead traversing the tricuspid valve
- CIED leadsn = 104
Main finding
As reported in the abstract
- comparable between those with and without CIED
Observational data: associations, not effects.
Further findings
Mild or less residual tricuspid regurgitation
Moderate or greater paravalvular leak
- Mild or less residual tricuspid regurgitation: P < 0.041
- Moderate or greater paravalvular leak: P < 0.017
Safety
| Outcome | CIED leads | without CIED |
|---|---|---|
| Lead failure · median follow-up time of 183 days | 5.8% | Not reported in the abstract |
Lead failure · median follow-up time of 183 days
- modest changes in pacing thresholds
Adverse events or mortality · 30 days
- No significant increase in adverse events or mortality
