Evidence for concealed fasciculo-ventricular connections as revealed by His bundle pacing

EuropacePublished Mechanistic study

Authors' conclusion

Our histologic findings confirm fasciculo-ventricular connections, initially described by Mahaim as being paraspecific, are likely ubiquitous. Analysis of 12-lead electrocardiograms leads us to conclude that fasciculo-ventricular pathways, concealed during sinus rhythm, become manifest with His bundle pacing.

Study context

Population
Serially sectioned histological datasets and individuals undergoing His bundle pacing
Participants
22 vs 34
Comparison
Histological datasets and ECGs during His bundle pacing
  • Histological datasetsn = 22
  • His bundle pacingn = 34
Outcome
Specialised fasciculo-ventricular pathways (histology) and QRS resembling fasciculo-ventricular conduction (delta wave) during His bundle pacing

Main finding

As reported in the abstract

Patients undergoing His bundle pacing with QRS closely resembling known fasciculo-ventricular conduction

32 of 34

Patients with no delta wave at any pacing voltage
