Hypnotic use and the risk of cardiovascular diseases in insomnia patients
- Prospective cohort study with drug-target Mendelian randomization analysis
- propensity-score matched
- UK Biobank
Authors' conclusion
Our findings suggested the heterogeneous associations between different categories of hypnotics and incident cardiovascular events in individuals with insomnia. Both observational and genetic evidence raised safety concerns regarding the cardiovascular impact of benzodiazepines. No cardiovascular hazard of Z-meds was discovered in the UKB population with insomnia.
Study context
- Population
- UK Biobank participants with insomniamatched cohort
- Exposure
- Regular hypnotic use vs no use
- Outcome
- Incident CHD, HF, stroke and CV death
- Follow-up
- Median 14.3 years
Main finding
As reported in the abstract
Matched cohort events: Coronary heart diseases (CHD)
929
Matched cohort events: Heart failure (HF)
360
Matched cohort events: stroke
262
Matched cohort events: CV deaths
180
Observational data: associations, not effects.
Further findings
Sensitivity analyses (IPTW, competing risk, shared frailty models)
- Did not alter the associations
Drug-target Mendelian randomization (MR)
- Corroborated CV safety of Z-meds in the general population
