Cohort study
Impact of Pulmonary Hypertension on Outcomes After Transcatheter Tricuspid Valve Edge-to-Edge Repair
- Registry-based cohort study
- multicentre registry
Authors' conclusion
PH is an important outcome predictor in patients undergoing T-TEER for relevant TR. In contrast to previous studies, no significant differences were observed for patients with precapillary and postcapillary PH in terms of survival free from HFH.
Study context
- Population
- Patients undergoing T-TEER for relevant tricuspid regurgitation (TR) (EuroTR registry, 2016–2023) with invasive systolic pulmonary artery pressure (sPAP) by right heart catheterization
- Participants
- 1,230 patients
- Exposure
- Elevated vs lower sPAP; precapillary vs postcapillary pulmonary hypertension (PH)
- sPAP >46 mm Hgn = 526
- Outcome
- Combined death or HF hospitalization (also procedural TR reduction and NYHA class improvement)
- Follow-up
- 2 years
Main finding
Death or HFH
Hazard ratio
1.027
95% CI 1.003–1.052
P = 0.030
Increasing sPAP
HR 1.027 (95% CI 1.003–1.052); P = 0.030
As reported in the abstract
Observational data: associations, not effects.
Further findings
Optimized sPAP threshold for prediction of death or heart failure hospitalization (HFH) (sensitivity analysis)
- Optimized sPAP threshold
- 46 mm Hg
- Elevated sPAP (>46 mm Hg) observed
- 526 patients (42.8%)
- Elevated sPAP associated with more severe HF symptoms at baseline and follow-up
NYHA class and TR severity
- NYHA class significantly improved and TR severity significantly reduced irrespective of PH
