Incremental Prognostic Value of Biomarkers in PARADIGM-HF
- Secondary analysis of the PARADIGM-HF randomised trial
- prognostic biomarker analysis
Authors' conclusion
None of the biomarkers studied individually or collectively led to a meaningful improvement in the prediction of outcomes over what is provided by clinical, routine laboratory, and natriuretic peptide variables.
Study context
- Population
- HF patients in PARADIGM-HF with biomarker measurements
- Participants
- 1,559 patients
- Comparison
- PREDICT-HF prognostic model with vs without candidate biomarkers
- Primary endpoint
- Improvement in PREDICT-HF model performance (discrimination, reclassification) for the primary endpoint, CV and all-cause mortality
- Follow-up
- Mean 30.7 months
Primary result
As reported in the abstract
Incremental prognostic value of biomarkers over PREDICT-HF
- Individually, only high-sensitivity troponin T (hs-TnT), GDF-15, cystatin C and TIMP-1 independently associated with all outcomes
- All biomarkers added together: only hs-TnT independent predictor of all three endpoints; GDF-15 also predictive of primary endpoint; TIMP-1 the only other predictor of CV and all-cause mortality
- No significant improvement in discrimination or reclassification, individually or in combination
