Timing of fractional flow reserve-guided complete revascularization in patients with ST-segment elevation myocardial infarction with multivessel disease: Rationale and design of the OPTION-STEMI trial

Am Heart JPublished Design of a randomised controlled trial

Study context

Population
STEMI patients with multivessel CAD and ≥1 non-IRA lesion with ≥50% stenosis by visual estimation; 50%-69% lesions assessed by fractional flow reserve (FFR), ≥70% revascularized without FFR
Planned comparison
Immediate vs in-hospital staged FFR-guided complete revascularisation
Primary endpoint
Composite of:
  • All-cause death
  • Non-fatal MI
  • All unplanned revascularization
Follow-up
1 year after randomization
Methodological detailsmulticentre, investigator-initiated, prospective, open-label, non-inferiority, 1:1 randomisation, Enrolment December 2019 to January 2024; primary endpoint follow-up to be completed January 2025, results expected mid-2025
