Artificial intelligence for individualized treatment of persistent atrial fibrillation: a randomized controlled trial

Nat MedPublished Randomised controlled trial

Authors' conclusion

These results show that AI-guided ablation of spatio-temporal dispersion areas in addition to PVI is superior to PVI alone in eliminating AF at 1-year follow-up in patients with persistent and long-standing persistent AF.

Study context

Population
Patients with drug-refractory persistent atrial fibrillation (AF)
Participants
187 vs 183
Comparison
Artificial intelligence (AI)-guided tailored ablation vs pulmonary vein isolation (PVI) only
  • Tailoredn = 187
  • Anatomicaln = 183reference
Primary endpoint
Freedom from documented AF with or without antiarrhythmic drugs after a single ablation procedure
Follow-up
12 months
Methodological detailsmulticentre, double-blind, superiority
  • Multicentre
  • Double-blind
  • Superiority

Primary result

As reported in the abstract

P < 0.0001 for superiority

Secondary outcomes

Freedom from any atrial arrhythmia after one ablation

No significant difference between arms

Safety

Composite safety endpoint (death, cerebrovascular events or treatment-related serious adverse events)

Did not differ between arms; procedure and ablation times twice as long with tailored ablation
