Left Ventricular (LV) Structure and Function in HFrEF With and Without Peak-Flow-Triggered Adaptive Servo-Ventilation-Treated Sleep-Disordered Breathing
- Secondary analysis of the ADVENT-HF randomised trial
Authors' conclusion
Among patients with heart failure with reduced ejection fraction, those with OSA had similar LV structure and function to those without SDB. Patients with CSA had greater LV remodeling and systolic dysfunction than those with OSA or without SDB. Six months of SDB suppression of OSA and CSA by ASVPF had no impact on LV structure or function.
Study context
- Population
- Patients with HFrEF (EF ≤45%) with or without sleep-disordered breathing (SDB)549 with SDB in randomised cohort
- Comparison
- Obstructive sleep apnea (OSA) vs central sleep apnea (CSA) vs no SDB; ASVPF vs control in SDB
- no-SDBn = 56reference
- OSAn = 543
- CSAn = 201
Primary result
As reported in the abstract
LV structure and function by SDB type
CSA had greater LV mass index and lower LVEF than the other groups
P < 0.05
LV volumes higher in CSA than OSA
P < 0.05
Further findings
LV structure and function with ASVPF vs control · 6 months post-randomization
- ASVPF abolished SDB but had no significant effect on LV structure or function, by allocation or adherence, in the entire cohort or either subgroup
- entire cohort n = 549
