Prevalence of Heart Failure with Preserved Ejection Fraction (HFpEF) in Isolated Severe Secondary Tricuspid Regurgitation

JAMA CardiolPublished

  • Retrospective cross-sectional study
  • single-centre
  • consecutive patients

Authors' conclusion

The findings suggest that HFpEF is underdiagnosed and should be rigorously evaluated for in patients with severe isolated STR, along with precapillary PH, as both have distinct requirements for management.

Study context

Population
Adults with severe isolated secondary TR (no EF <50%, left-sided valve disease, congenital, infiltrative/hypertrophic cardiomyopathy, pericardial disease or prior cardiac procedures) undergoing rest-and-exercise right heart catheterization (RHC), Mayo Clinic, 2006–2023
Participants
54 adults
Outcome
Prevalence of HFpEF and precapillary pulmonary hypertension

Main finding

As reported in the abstract

HFpEF identified

40 patients (74%)

HFpEF recognised before RHC

19 patients (35%)

Precapillary pulmonary hypertension (PH) among the 14 without HFpEF

10 (71%)

Observational data: associations, not effects.

Further findings

OutcomeLeft atrial emptying fractionLeft atrial strain
Discrimination of noninvasive measures for HFpEF
AUC0.90 (95% CI, 0.82-0.98)0.91 (95% CI, 0.83-0.99)

Guideline-defined diastolic dysfunction absent in patients diagnosed with HFpEF

Diastolic dysfunction (DD) absent
