Stressed Blood Volume in Severe Tricuspid Regurgitation: Implications for Transcatheter Treatment

JACC Cardiovasc IntervPublished Observational cohort study

Authors' conclusion

In patients with severe TR, eSBV is associated with obesity, renal and liver dysfunction, more severe heart failure, attenuated reduction of venous congestion after TEER, and adverse clinical outcomes.

Study context

Population
Patients with severe tricuspid regurgitation (TR) undergoing right heart catheterization prior to tricuspid transcatheter edge-to-edge repair (TEER)
Participants
279 patients
Exposure
Higher vs lower estimated stressed blood volume
Outcome
Associations of eSBV

Main finding

As reported in the abstract

Estimated stressed blood volume (eSBV) associated with obesity, renal and hepatic dysfunction and cardiac remodeling

P < 0.05 for all

eSBV correlated with pulmonary artery and cardiac filling pressures and RV-pulmonary artery coupling

P < 0.05 for all

After TEER, eSBV > median: less reduction in right atrial pressures, peripheral edema and ascites vs lower eSBV

P < 0.05 for all

Higher eSBV independent predictor of death and HF hospitalization

P < 0.05 for all

Observational data: associations, not effects.
